Well, anime is supposed to be taken at face value. Or any entertainment medium for that matter. Because discussing the "what-if" scenarios for fictional characters are pretty much useless because the authors can bend it any way they want. So there is no correct answer to your question unless you have read the entire LN where it is at least implied that they had sex at some point. Some ppl might say yes and some might say no. But the main thing is what you do with each answer.

Best ending line in anime history = "My name is Saiki Kusuo. I am a psychic."

Apparently they are step siblings.
I don't think anime ever goes in depth of their relationship, but they are quite literally inseparable, which means it is unlikely that they would do the deed with anyone else.
