Anyone knows what "ef" means?

Probably Ef stands for Eternal Feather (Yuukyuu no Tsubasa, the theme song of the visual novel).
It may also be Euphoric Field (opening of the anime), but Eternal Feather makes more sense since the original visual novel came before the anime.

If you watch the opening theme to Ef- a Tale of Melodies it tells you why/ all the possibilities to why it is called ef. In a few frames the phrases 'Eternal Feather', Emotional Flutter', 'Euphoric Field', 'Ever Forever', and 'Ebullient Future' appear.

btw, iirc there's an interview with Minori's staff and they say it "could mean a lot of things".
So all of these are possible matches in order of coherence:

Eternal Feather (OP song for Ef: The First Tale)
Emotional Flutter (OP song for Ef: The Latter Tale)
Ever Forever (ED song for Ef: The Latter Tale)
Echt Forgather (Insert music PS2 game)
Euphoric Field (OP song for the Memories)
Ebullient Future (OP song for Melodies)

Although, after playing both versions of the VN (NSFW and PG-13) I noticed they strongly emphasize the "Ever Forever" thing with Yu-kun and Yuuko, so I'd bet my chips on that.

About the catch with the episodes titles, it's SHAFT stuff.
And they did it again with season 2.
The first letter of episodes 1~6 spells "erutuf" that backwards is "future" and the following eps adds a "redo" negative/affirmative in front of it till finally ending in ep 12: "forever".
