Since volume 10 just came out I can summarize that:

[SPOILERS FOR KOKORO CONNECT VOLUME 10]

Basically, the CRC force themselves into the isolation zone, at first, Taichi and Nagase can't find any of the others, but eventually they show up too and they band together. It's revealed Heartseed couldn't make it in after all, and that any fights or bonds being broken results in that person disappearing from the isolation zone and losing their memories, and if this happens too many times emergency shutdown would happen and everyone would lose their memories.

The Student Council President Katori tries to take control of the situation while the CRC try to calm everyone down so an emergency shutdown wouldn't happen, but they have to be careful not to reveal too much so The Third ignores their presence, and pretend to also have a phenomenon like everyone else. The Second is an ally now who tells them this, but not a reliable one.

Soon, Heartseed appears and he tells everyone that the solution is the unite everyone in the Isolation Zone so no new developments can happen, in which case The Third will shut down the game.

So the CRC try to band everyone together but it goes horribly wrong, Katano investigates and finds out that fights is what's causing students to disappear from the Isolation Zone and declares that as their best method to escape. All the students, now desperately wanting to escape, start to sway over to Katano's side and want to start fights to escape, not knowing it erases their memories. Katano tries to rationalize that their memories only briefly vanish and come back when they return to the real world.

Obviously, the CRC try to stop them, but they get found out for having no phenomenon and distrusted by everyone around them and fights break out even more. Just as it seems everything is hopeless, a student speaks out in support of them and Fujishima Maiko also delivers a speech on why they shouldn't disappear and instead unite.

Eventually Katano gives up, and all the students do band together, and the Isolation Zone falls apart.

But at the last second, Heartseed tells them that the CRC are gonna lose all memories of their phenomenon regardless, at the last moment as everything is wiped away, Taichi shouts out that he's glad Enjouji and Chihiro joined the club, that he's grateful to Aoki and Yui for being his friends, that he doesn't regret falling for Iori, and that he loves Inaba.

A few weeks later, it's shown that the CRC have lost their memories. They all feel like something is missing, and the notebooks and clues and photo albums they left behind before entering the Isolation Zone are all blank. Everything has been reset.

Until, Heartseeds get bought up and everyone is unsettled by that word, and they all collectively realize that they're all freaked out by the word Heartseed. Then, Iori finds a map inside her bag to the hospital. Wanting to investigate, they all go, though Inaba is reluctant.

Over there, Iori remembers the climax of the body swapping arc and hugging Taichi, though she doesn't remember the context and why it happened. Eventually, Yui and Aoki visit another place and regain some of their memories, and Inaba takes Taichi to the place where they went on the school trip and remembers everything that happened with Iori and her feelings for Taichi.

Eventually, they all meet up again. Taichi brings with him the matching pendant that he has with Inaba, Inaba shows him his pendant too, and Taichi tells her that he loves her. At this, the phenomenon breaks and everyone regains their memories.

Heartseed appears and tells them that this was part of his plan to throw off The Third, since if they just pretended to lose their memories he would catch on, so he let their memories be erased and relied on the hope that they would regain it. And purposefully pretended like he betrayed them to leave a stronger impression and give them the drive to regain their memories.

After this, everyone thanks Heartseed for leading them on to the path they are on today, and he finally leaves forever.

Finally, there's an epilogue, where they're all at the start of their third year and living happily, finding out what classes each of them are in, and then Taichi finds out him and Inaba are in the same class, and she appears, he finally calls her by her first name, Himeko.

The final few lines of the series are about the power of human connection, and how important and world changing it is.

Great summary of V10, thanks for sharing.
Good to know even though after rewatching the anime, I don't like or respect it as much as I thought I did years ago, even if the themes are fairly good.

For anyone curious, Vol 11 is an extended epilogue, mostly featuring a series of short stories after Vol 10.

The short stories revolve around Rina (Taichi’s sister), Fujushima and her usual love related shenanigans, a first year student looking to join the CRC after the OG 5 leave during third year and the final story in the book is from Iori’s POV about what she’s going to do in the future. Pretty solid ending.
